I''ve been given this case to solve in an attempt to understand the topic of "PATIENT CLINICAL DATA ANALYSIS" to develop my competency in reading and comprehending clinical data including history,clinical findings,investigations and comeup with Diagnosis and Treatment plan.

A 65 years old male came to OPD with
Chief complaints of: swelling in abdomen and cough since 10 days

History of present illness: patient was asymptomatic 10 days ago later he developed swelling in abdomen and break in urinary stream and he also had cough since 10 days and he has swelling in the region of neck since 10 years  with this symptoms he visited nalgonda govt hospital he adviced to visit Kim's for dialysis

Marital status: married 
Occupation: farmer
Appetite: normal
Diet: mixed
Bowel : regular 
Micturation: abnormal 
Addictions: alcoholic and smoker 20 years back

Conscious,  coherent, co-operative Moderately built, nourished.

 EDEMA Of FOOT : PRESENT

RESPIRATORY RATE: 28 Cycles/min
BP:180/100mm of hg 
SPO2:98% at room air

INVESTIGATION
DIAGNOSIS: bilateral pleural effusion
Grade2 rapid pelvis dilation in bilateral kidneys with bilateral cortical cyst

HOPI :  patient was apparently asymptomatic 10 years ago ,then he developed pain in the right loin for which he diagnosed to be having renal stones and got  operated for it .  Hi, iam maheshwari  of 3rd semester. This an online E logbook to discuss our patients health data after taking his consent.This also reflects my patient centered online learning portfolio. The patient’s consent was taken prior to history taking and examination of his condition. Cheif complaints: A 47 year old female which cheif complaints of pedal edema shortness of breath & abdominal distension and cervical lymphadnopathy  HISTORY OF PRESENT ILLNESS: patient was apparently asymptomatic 6 years back and had generalized body weakness for which she went to hospital and diagnosed it as diabetes mellitus 2 and was on regular medication . 2 months back patient had pedal edema below hip ,pitting type and abdominal distension for which she visited near by hospital and was diagnosed hypotension and hypothyroidism after necessary test pedal edema was subsided on medication but abdominal distension was not subsided for which she came to her hospital and was admitted and treated for 1 weak f
